    This is the individual (21+ yrs of age). who is responsible and authorized to register, purchase and attest to our rental agreement.

    The password must have a minimum strength of Medium    Strength indicator

    The following information is required. 1, Approx. number of guests (Max 75). 2. Drivers License or Government ID image file to be uploaded in the field provided below and associated with this rental. 3. Completion of the rental agreement indicating you attest to reading and agreeing to the Ewing PBA Local 111 Hall Rental Agreement - NOTE: Do not check the Terms and Conditions acceptance unless you have read SIGNATURE TYPE SIGNED SUBMITTED the full agreement form electronically on the agreement page. You should do that first HERE before checking the Terms and Conditions box below which attests that you have read and digitally signed and accept the contract. Questions or concerns?   • Upload NOTE: Create a photo of your driver's license/ Uploading the file here is a requirement in order to register and book any PBA Hall Rental. This photo and completion of the Rental Agreement Form below is a requirement for all PBA Hall bookings. (File formats: .jpg .jpeg .png .gif only)
